Onimusha Way of the Sword best hozuki pouch choice depends on one thing: whether you have learned the timings yet. Run the Default pouch while you are still reading attacks, then make Regeneration your everyday choice once your parries and deflects are reliable. There are five pouch types, only one is equipped at a time, and you can swap freely and for free at any Spirit Mirror, so there is no permanent wrong choice.

Onimusha Way of the Sword best hozuki pouch: the five types

PouchWhat it restoresCharges at Lv3Use it for

Default

Instantly restores health, roughly a quarter of max health at top level

7

Learning enemy timings, and any time you need HP right now

Regeneration

Gradually restores a large amount of health and also restores stamina

7

The general-purpose pick once your timings are reliable; long boss fights and chip damage

Resistance

Gradually restores health and Oni Power

6

Aggressive boss attempts where you want to stay offensive but still recover

Aggressor

Gradually refills Oni Power and the Reflex Gauge, with little to no healing

4

Aggressive players who dodge reliably and want more Oni Armament attacks

Awakening

Fills Oni Power and charges the Oni Awakening gauge, with no healing

3

Endgame and Carnage runs; pairs well with Still Water

Charges are the real limiting factor. The two most offensive pouches have the fewest uses, so they are not worth rushing to, and the Aggressor pouch does nothing in a crisis where you expected healing.

Why the choice changes as you get better

Default is an instant heal, so it is wasted if you are about to be hit again — which is exactly what happens early, when you are still eating follow-ups. Once you can create an opening before healing, a gradual heal that also returns stamina becomes strictly better, and Regeneration is the only pouch that gives stamina back. That matters because panic-blocking drains stamina and leaves you open to punishment.

The two aggressive pouches trade healing for offence. Pick them when your defence is already dependable and you would rather spend the fight attacking, which is the endgame situation rather than the first-run one.

Charges refill at Spirit Mirrors or by using Sun Dried Hozuki items.

Unlock and upgrade

You start with the Default pouch, and pouch upgrading opens after completing the mission "Patching Up the Pouch", which also marks all four Oni Cloth locations on your map. Okuni at Rokudo-chinnoji in Enma's Hall performs the upgrades. Each pouch has three levels: level 2 uses ordinary materials, and every level 3 needs one Skeleton Hozuki, of which there are five. Maxing all five to level 3 unlocks the Medical Marvel trophy.

Planning around the five Skeleton Hozuki

The Awakening pouch stays locked at level 1 until Shuten Doji is defeated, so save one Skeleton Hozuki for it rather than spending all five early. Hidden Treasure No. 5, one of the five Chance Encounters, contains a Skeleton Hozuki and is the one most players miss.

There are nine Hozuki Pouch pickups in the game that raise capacity, and ordinary Hozuki plants that overflow your pouch go to the Storehouse and refill it for free between Spirit Mirrors. So pick up every plant you pass, and hoard the rare Sun Dried Hozuki for boss attempts rather than spending them on exploration.

Charm of the Healer increases the value of every pouch use, including those free refills, so the two upgrades reinforce each other — both are on the charms guide.

Because only one pouch is equipped at a time, the swap is the real upgrade. A pouch that is wrong for the run can be changed at any Spirit Mirror for free, so the level-3 material cost only matters for the pouch that will actually be equipped — and only one of them needs all five Skeleton Hozuki.

When a charge is worth spending

A charge is worth spending in a window where you will not be hit again for a moment, and only then. Default heals instantly, so it is the one to reach for the instant after a hit when the boss is already in recovery — spend it late in a string and you heal straight back into the next hit. The gradual pouches are the opposite: they pay off most when used before a long stretch of pressure, so fire Regeneration or Resistance at the start of a boss phase rather than the end of one.

The two offensive pouches are the exception to the "heal early" rule. Aggressor and Awakening restore no health at all, so they are not a panic button and should be used in advance, while the enemy is still recovering and Oni Power carries you into the next string. The build these feed is on the build guide.
